Not sure where are you reading those stuff from but the only issue you're having with maphack is that you haven't been following the guide, it has nothing to do with the MPQ patch since items codes are all original and file structures are exactly the same everywhere otherwise the game wouldn't work properly anymore. As you've already been told here, you shouldn't even touch the .vcb file inside of the sting maphack in the first place so just read the guide and only edit d2hackmap.cfg file and nothing more.
