I think is the most popular and usefully setup.

 

Well, about puting JAH into stuff for more life... One should do a little math, if the JAH will be best in the slot.

+% to life bonuses stack, so it counts like this:
your hard life * (% of BO + % from JAH(s) + % of OAK(if there is one))

so if you count it like this (to make it simple)
1000 hp * (100% BO + 10% JAHS (e.g. eni + shako) results into 2100 hp
1038 hp (p ruby in shako instead of jah) * (100% BO + 5% JAH (eni)) results into 2128 hp

for paladin with 2200 hp prebo and with lvl 15 BO, it will be like this:
2200 hp * (77% BO + 10% JAHS) = 4136 hp
2238 (pruby) * (77% BO + 5% jah) = 4073 hp

As you can see, if you have low BO and quite a lot of hard hp, its better to get some JAHs into sockets. On the other hand, if your starting HP pool is low, its always better to go the P ruby way.

 

Well, I personally would prefer 125 fcr on hammerdins everytime. It feels more comfortable to me.

Hoto, to gloves, arach, spirit, 10fcr 2pala amu, soj/bk's will do the trick - or use maras and 1 fcr ring for lower dmg but more life due to higher bo level. You will lose dmg but you get 1 more frame which results in higher killingspeed.
If you want to use Zaka for better defense (which is only necessary in pvp imo) you can run 20fcr circlet, 15fcr crafted Amulet, 1 fcr Ring. This is the highest possible dmg on 125fcr setup but will cost alot depending on circlet/Amulet stats. It depends how much you wanna invest for it.

And about the life - bo thing. You get the most life running shaco and lvl20 bo (125fcr with fcr ring + maras) or lvl21 bo (80+fcr). Do the maths as Quinneczech pointed out.
